SIPOC templates

The Six Sigma scoping diagram — Suppliers, Inputs, Process, Outputs, Customers — as five aligned columns with the process steps as a spine across the middle. Optional `requirements:` rows attach CTQ criteria to inputs and outputs, so the chart that usually says what a process touches also says what 'good' means for each one. The first artefact of every DMAIC define phase.

  • SIPOC — Inpatient Discharge

Scopes ward discharge from the consultant's decision to the released bed across six steps, naming the three inputs and two outputs that carry no stated requirement and measuring the 7.5-hour decision-to-departure time against its 4-hour target.

  • SIPOC — Injection Moulding Cell

Fixes the boundary of a seven-step moulding cell from resin release to finished-goods booking, checking both scope ends against the process and scoring three tiles — 3.2% scrap against a 1.5% target, 47 s cycle against 42 s, 34-minute changeover against 20.

  • SIPOC — Sev-1 Incident Response

Written in the terse s/i/p/o/c dialect with a prose scope sentence, this map bounds payments incident response at seven steps and shows the 68-minute mitigation time missing its 30-minute target while acknowledgement beats its own.

  • SIPOC — Cold Chain Vaccine Despatch

    Operations & Ops
  • SIPOC — Undergraduate Enrolment

Uses a numbered continuation list for its six process steps and a prose scope sentence, showing that four of the five enrolment outputs carry no stated requirement while only 81% of students register by day one against a 95% target.

    Operations & Ops
  • SIPOC — Offshore Turbine Service

Declares its critical inputs with the ctq shorthand rather than a trailing marker, bounding a seven-step offshore service window and computing the 23 technician hours against a 16-hour target and 78% right-first-time against 90%.

    Lean & Flow
  • SIPOC — Householder Planning Permission

Scopes a local authority planning determination at six steps from validation to decision notice, reporting the two inputs and two outputs without a stated requirement behind a 71% eight-week determination rate and a 47% invalid-on-submission rate.

    Business & Strategy
  • SIPOC — Omnichannel Apparel Returns

A deliberately mis-bounded charter: the scope ends at the refund but the process carries on to restocking, so the engine confirms the start boundary and flags the end as running past its own limit alongside an 8-day refund cycle against a 3-day target.

    Operations & Ops
  • SIPOC — Clinical Trial Site Activation

Bounds phase III site start-up at seven steps from site qualification to first patient screened, with three of its six inputs carrying a stated requirement and the 137-day activation cycle measured against a 90-day target.

    Project & Planning

    Lean & Flow
  • SIPOC — Narrowbody Aircraft Turnaround

Bounds a 25-minute A320 turnaround at eight steps from on-blocks to off-blocks, showing 34-minute actual turn time and a 71% on-time departure rate against the inputs nobody wrote a requirement for.

    Operations & Ops
  • SIPOC — Clinical Coding to Invoice
